We now offer a program that provides you with an on-screen HCFA 485, 486 and 487 that you can fill in and print. Updated to refect CMS. Available for Windows and requiring that you have Microsoft Word installed on your machine, you can easily complete and print as many copies as needed of the forms, after you fill them out on screen. Works with Inkjet, Laser and Tractor Feed printers, since it generates the entire form with the data.

Each form can be printed and saved. Data on previously saved forms can be used to do updated forms, making the process very fast and easy.
